Southlake is excited to announce new opportunities in our innovative Extensivist Clinic, opening in early 2025. A first-of-its-kind, intensive outpatient care program designed to deliver leading-edge, patient-centered care. This Clinic is a collaboration of a dedicated inter-disciplinary team, including Nurse Practitioners, RNs, RPNs, Social Workers, Pharmacists, and others, to ensure comprehensive patient care.
The Extensivist Clinic will provide integrated care for patients to prevent unnecessary admissions and ensure safe, earlier discharges with coordinated follow-up. It provides a broader approach to sustaining patient health and stability over time.
Building on lessons learned from the successful Southlake@home program which has now spread across the province, this Clinic serves as a critical bridge between inpatient and outpatient care and community services. It offers stabilization and ongoing follow-up for patients Located on our hospital campus in Newmarket, the Clinic takes advantage of Southlake’s robust resources and expertise to enhance patient outcomes.

Clerical Support is a member of the team who works with the support of, and in collaboration with, all health care providers within the Department. The Program Clerical Support is responsible for reception, patient scheduling, word processing, and clerical functions, reporting to the Department Manager. He/she role models the organizational core competencies.Primary ResponsibilitiesWorks under the direction of nursing staff and physicians to coordinate patient appointments and perform clerical duties. Health Program Clerical Supports also act as receptionists and communicate with patients on the phone and in person.Skills & Abilities

  • Minimum one (1) year of clerical duties in an acute care health care environment required
  • Proven ability to attend work regularly
  • Demonstrated current knowledge and skill in the use of medical electronic health record systems
  • Current experience in computerized provider/physician order entry (CPOE) an asset
  • Proficiency with Microsoft Office preferred
  • Demonstrated willingness to learn/innovate
  • Commitment to customer service and teamwork
  • Demonstrates analytical skills in relation to knowledge of hospital operations
  • Effective organizational and prioritizing skills

Qualifications

  • Minimum Grade 12
  • Medical Terminology Certificate required
  • Diploma in Medical Office Administration OR evidence of relevant and related post-secondary diploma from an accredited academic institution preferred
