BSO Strategic Lead

The HNHB Behavioral Supports Strategic Lead is a regulated health professional who will serve as a leader, coach and project manager for the implementation, expansion, integration and evaluation of the HNHB Behavioral Supports Action Plan. The BSO Strategic Lead will work with Ontario Health West, HNHB providers and other OH Regions to advance the BSO Framework.The HNHB BSO Strategic Lead enables systemic, cross-sector quality initiatives through facilitating a virtual team of formal and informal BSO Leaders from various organizations and the HHS BSO Coordinator, and encouraging stakeholder involvement. Relationship-building and engagement enable the Lead to facilitate sustainability of the models through ongoing identification of opportunities; examining cross-sector system-level opportunities to make change (LTC, Hospitals, Community, Primary Care); initiating quality improvement initiatives; exploring new ways of system re-design and elevating the systemic barriers to the steering committee and/or HNHB.Key responsibilities:
– Provide leadership and project management to support the implementation and integration of the HNHB BSO Action Plan and other activities identified by the HNHB Behavioral Support Management/Advisory Committee(s) and Ontario Health West
– Provide direction for HNHB Behavioral Supports initiatives to ensure their alignment with the HNHB BSO Action Plan, the BSO Framework, and the OH West Strategic Health System Plan
– Identify and facilitate opportunities to expand HNHB BSO service models to incorporate other populations that exhibit or are at risk of Responsive Behaviors
– Establish relationships and open consultation with HNHB providers and other key stakeholders, including Health Quality Ontario, Alzheimer’s Knowledge Exchange and BSO Coordinating Reporting Office (CRO) or other Provincial stakeholder groups
– Facilitate regular meetings of the HNHB BSO Management/Advisory committee and meetings with other stakeholders as needed
– Establish tracking processes to monitor implementation of each project improvement initiative, including identification and/or development of tools to measure success and ongoing monitoring
– Monitor and guide implementation, prepare project reports and initiate corrective action as needed
– Submit quarterly reports and ensure deliverables and time lines are met
– Operate within the HNHB Behavior Supports Governance Framework
– Work with OH West to integrate HNHB BSO strategies with existing and new service models that provide care/services to shared clients
– In partnership with providers, identify processes to identify and translate best practice from one sector to another
– Interface and work collaboratively with the Ontario Health West on local and provincial evaluation processes, human resource issues/plans, training, knowledge of exchange and other initiatives
– Responsible for one full time HNHB BSO Coordinator and one full time BSO Clinical Leader employed at Hamilton Health Sciences
– Work with the Director of Community and Population Health Services and provide program updates and reporting as required
– Manage stakeholder relationsOutreach to all health sectors is a pivotal role for the Lead, having the skill to build relationships, understanding both the clinical and systemic levels to make sustainable change, skillfully helping stakeholders make the connections and identify opportunities for improvement within existing resources, and providing meaningful, engaging presentations that enable setting the stage for expectations of providers that align with the HNHB Strategic direction and initiatives.The role entails the ability to understand the clinical, local, system and strategic levels of understanding the supports for the BSO population in order to build on the expertise and successes of the providers, while at the same time, working to make improvements that shift the culture and may, respectively, challenge the status quo.Qualifications1. Healthcare Professional, or clinical experience, with 5-7 years experience minimum in multiple sectors
2. Post-secondary degree in a health professional field, health administration, health planning or relevant field
3. Proven ability to build relationships and lead stakeholder engagement with diverse populations, cross-sector (hospital, long-term care, community, primary care)
4. Engaging and dynamic presentation skills that integrate multi-faceted aspects of care
5. Proven ability to deliver under tight deadlines with effective time management skills
6. Experience with quality improvement, evaluation, project management
7. Experience leading and managing a team to successfully achieve outcomes of a complex nature.
8.Leadership, facilitation, mentoring, dispute resolution skills
9. Self-starter who can work with limited guidance
10. Well-developed analytical and problem solving skills
11. Excellent verbal and written communication skills
