Since 1949, TVCC has been working with children and youth to help them reach their full potential. Today, a wide range of services and supports are provided for young people with physical disabilities, communication disorders, developmental delays and autism spectrum disorders to support them in attaining the best available quality of life and health. In partnership with local and regional agencies, TVCC annually serves more than 11,000 children, youth and their families from birth to age 19 across Southwestern Ontario in over 700 locations.We are currently recruiting for a temporary, full time (37.5 hours per week) Clinical Coordinator to support School Therapy Services. This position will support School Therapy Services staff and work closely in collaboration with the other Clinical Coordinators and Managers. The designated office for this position is London.This position is expected to begin in November 2024 and end in November 2025.Responsibilities:
- Coach and mentor clinicians in development of clinical expertise in relationship centered practice, clinical decision making and practices;
- Support adherence to program procedures and clinical processes;
- Assist in orientation and training of staff;
- Support clinicians with the management of workload and caseload to achieve productivity standards and expectations of service delivery;
- Assign referrals to clinicians,
- Monitor wait for service and other workflow data and work with Manager to identify appropriate resources and service delivery strategies;
- Interpret and apply TVCC standards, policies and procedures and support adherence to all expectations (e.g. chart audits);
- Work with other service providers and partners such as TVCC programs, School Boards, Ontario Health atHome, LHSC, and/or other children’s service agencies to support integrated services;
- Ensure continuum of care through caseload transitions, intake, and service pathways;
- Facilitate development of service delivery best practices for the clients and families and family-centred communication and documentation practices;
Qualifications:
- Minimum 4-year undergraduate degree in health care, social sciences or a related field; membership in the appropriate professional college or board; and demonstrated knowledge of applicable standards of practice;
- Minimum three years of clinical experience in pediatrics;
- Demonstrated leadership, coaching and mentoring skills;
- Critical thinking skills and the ability to resolve conflicts and negotiate outcomes;
- Strong interpersonal skills ,demonstrating a reflective practice and the ability to receive and incorporate feedback;
- Demonstrated ability to manage and prioritize multiple demands efficiently and effectively to meet deadlines;
- Knowledge of the Life Needs Model of Service and a vision toward service improvements;
- Computer proficiency including MS Office Suite 365 package of software, especially Word and Excel, and videoconferencing software (ex. Teams)
- A valid Ontario driver’s license and access to a vehicle are required.
