The Indigenous Health Nurse Clinician (NC) will collaborate with Blood Tribe Department of Health (BTDH) to provide home based dialysis treatment for patients with end stage renal disease (ESRD). The purpose of this collaborative working relationship is to build comprehensive kidney care service models to support patients living within Indigenous Communities and specifically within the Blood Tribe Reserve #148.This position may extend to support other Indigenous health initiatives within Alberta Kidney Care South (AKC-S). The goal is to provide equitable and quality dialysis care closer to home by improving and expanding on reserve dialysis options through community health center programs. The aim is to improve the health care services for Indigenous populations by codesigning a variety of renal care initiatives to support patient and family needs while actively enhancing care in the community. The NC provides leadership in assessment, planning, implementation, and evaluation of care and modality education provided for the purposes of promoting, maintaining, or restoring health, and managing multiple comorbidities. A significant portion of care encompasses patient education to increase patient knowledge and informed decision-making in respect to the optimal treatment for kidney failure, given the patients’ circumstances, abilities, and/or physiological health factors. Where appropriate and consistent with the patients’ wishes, education will promote living donor transplantation, peritoneal dialysis, home hemodialysis, and non-dialysis conservative care management. The Indigenous Nurse Clinician will support quality improvement and project initiatives to support the Indigenous population within AKC-S.
As a Nurse Clinician (NC), you will be responsible for the coordination of an interprofessional team, in the provision of quality patient (client, resident) care in order to meet the operational priorities and requirements within a clinical setting. You will use an integrative leadership approach in assisting, planning, implementing and evaluating care needs, while assuming responsibility for staff assignments, patient flow and clinical resources. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP). Additional Required Qualifications:
Project management experience or expertise. Quality improvement experience. Demonstrated computer and electronic health record documentation skills required. Minimum of 3 years of recent or current nephrology and/or chronic disease management experience required. Ability to maintain effective and respectful working relationships required. Experience in the Case Management process of care with in the last 5 years. Demonstrated excellent interpersonal, organizational, assessment, and teamwork skills required. Ability to apply the principles of adult learning required. Experience and/or specialized training in diabetes, hypertension, cholesterol, and/or adult modality education principles. Experience in policy development and patient education materials.
Preferred Qualifications:
Baccalaureate Degree in nursing or other related degree. Canadian Nurses Association (CNA) in Nephrology Nursing. Experience as a RN in a chronic disease management setting within the last 5 years. Experience in leading group education for patients and staff. Experience in leading multidisciplinary rounds. Experience in data collection and analysis. Experience working with various stakeholders outside of Alberta Kidney Care South. Experience working with Indigenous populations.
