For more information, visit .POSITION SUMMARY:The Patient Navigator provides active leadership to facilitate and coordinate patient flow from admission to discharge, across acute and post-acute care areas in partnership with stakeholders. Using a collaborative approach and advanced communication skills, the Patient Navigator builds strong relationships with patients, families, care team members and system partners. The Patient Navigator play a key role as facilitator, communicator and change leader in support of both the daily management of patient flow (bed flow management, repatriations, eliminating barriers to specific flow challenges) and the improvement of processes supporting access, quality and safety.Specific processes within patient navigator scope can include the following:
To support process and system level improvement, the Patient Navigator contributes to and implements policies and procedures related to the care delivery process and patient flow strategies, assists in the development of quality and process improvement initiatives, participates in, and shares responsibility for, ensuring that performance is optimized, from the unit level, to the corporate level.The ideal candidate will have excellent interpersonal and communication skills combined with the ability to collaborate within and across professions; proven facilitation and conflict resolution skills; demonstrated ability to guide and influence others; experience with performance improvement knowledge and skills, preferably with experience in LEAN; ability to prioritize and manage resources, comprehensive knowledge of and experience with acute care hospital patient care processes.
