About the Position
As a valued member of the team, the PSW Quality Assurance Coordinator demonstrates a commitment to the quality of resident care by providing services in accordance with the organization’s vision, mission and values.
The PSW Quality Assurance Coordinator is ideally a certified Personal Support Worker and provides front line leadership to PSW’s and RSA team members, monitoring care standards within the LTC home. The PSW Quality Assurance Coordinator is responsible to the Director of Care for the overall quality improvement of resident care. This position is responsible for monitoring the quality outcomes as it relates to residents and the care provided by PSW’s. PSW Quality Assurance Coordinator acts also as the facilities PSW Education Coordinator and is responsible for monitoring PSW placements.
There will be a requirement to work one weekend a month with rotating shifts (days, evenings, and occasional nights) to evaluate PSW work and processes.
Responsibilities
Plans, supervises, and orientates new team members and conducts in-service education with nursing service personnel
Provides leadership, education, direction to PSWs to ensure standards of care are met
Monitors and evaluates PSWs, provides leadership and coaching on improvement goals and evaluates performance/provides input to the performance appraisal; identifies PSW learning needs and makes recommendations for appropriate education
Delegates responsibilities and duties to competent individuals, as required
Liaises with other disciplines to ensure interdisciplinary communications and teamwork for all team members
Establishes and maintains good working relationships with all personnel in the Home
Serves on committees as designated by the Director of Care; performs other related duties as required by the DOC
Ensures completion and analysis of monthly audits and indicators and identifies corrective action; investigates and implements improvements through audits, appraisals, and research
Ensures the incident reports are completed and forwarded for appropriate follow-up
Provides ongoing information to team members, fostering involvement, resident choice, and autonomy
Participates in the identification, analysis, and interpretation of trends in resident care outcomes to determine priorities for educational programming
Contributes to the planning, initiating, implementation, and evaluation of educational programs and learning resources for residents, family, and team members
Participates or provides input to the strategic planning process
Works with the Quality Committee to plan and implement new initiatives
Participates in orientation and performance management process in accordance with organization standards for all PSW team members in the nursing department
Ensures that team members is evaluated during probation and annually thereafter with measurable performance goals; ensures that all team members foster positive customer service with residents and families in their day-to-day work
Observes work patterns and ensure safe working practices are always adhered to
Implements and monitors zero lift policy
Ensures that all PSW team members are properly trained and aware of emergency fire and disaster procedures
Knows and interprets MOHLTC regulations and standards
Work in accordance with the organization’s Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act; conducts daily safety audits in the nursing department. Ensures nursing team members perform prestart up inspections and water temperature monitoring
Perform other duties as required
Qualifications:
Minimum of 2 years’ experience in Long-Term Care or equivalent working in a related position (i.e. PSW, Nursing) is required
Previous experience implementing quality standards in a health care setting would be an asset
Previous supervisory, coaching, mentoring and training experience
Excellent leadership and communication skills
Proficiency with the classification process and the funding indicators
Clinical knowledge and skill in gerontological nursing
Knowledge of MOHLTC regulations and standards
Effective verbal and written English communication skills required
Education:
Health-related diploma and/or degree would be an asset
Current Personal Support Worker certificate is required (or extensive experience working as a Personal Support Worker, in a Long-Term Care setting would be acceptable)
Working Conditions:
Providing full care to male and female residents of all ethnic and cultural backgrounds, including but not limited to: personal hygiene
Some disruption of lifestyle resulting from extra hours spent dealing with prospective clients, attending meetings, dealing with residents and families, and responding to emergencies
Stress resulting from constant interruptions, managing within legislative, budgetary, and time constraints on a consistent basis and dealing with people who are under emotional pressure
Combination of natural light and florescent lighting
Sitting for meetings and computer work
Crouching, kneeling, stooping/bending for short periods of time
Walking and climbing stairs frequently
Exposure to bio-medical waste
Engagement and/or exposure to various therapy programs focused on providing residents with the highest quality of life in our Homes including but not limited to pet therapy, spiritual events, and musical therapy
