Summary of DutiesTo provide a full range of social work services to adult Mental Health patients and their families. Services include psycho-education regarding mental illness, mediation and assistance in resolving conflict; ongoing consultation and collaboration with colleagues to coordinate optimal services for patients with an interprofessional approach; resource counselling – referrals to appropriate community supports; optimal discharge planning and follow up care arrangements.Qualifications/Skills
- Masters Degree in Social Work required
- Member in good standing of the Ontario College of SW
- Minimum 1 year of experience working as a MSW in Mental Health
- Commitment to Patient Centered Care
- Excellent communication and organization skills
- Strong team building skills
- Excellent assessment, interviewing and problem solving skills
- Knowledge of the health care system and community resources
- Superior ability to manage multiple tasks and set priorities in a fast-paced environment
- Commitment to student education
- CPI (Non-Violent Crisis Intervention) an asset
