If yes, we invite you to read the details below then apply.Hourly Rate Range: $61.51-$76.82Schedule Requirements: Days and some eveningsLocation: WilsonEmployee Group: ONAReporting Relationship: Manager, Schulich Family Medicine Teaching Unit & Integrated Care Systems.Job Summary:The Nurse Practitioner is a leadership role within a multi\disciplinary and inter-professional team. This Nurse Practitioner role in NLOT team is primarily responsible for providing wound care clinical support Long Term Care (LTC) homes. The NP works in collaboration and consultation with a physician, the LTC or congregate setting coordinators, and members of the inter-professional health care team to support complex wound care in the LTC and prevent ED visits. The NP will act as a link between hospital and the LTC homes to ensure the most appropriate and efficient care is received in relation to their wounds.Responsibilities:

  • Provision of Nurse Practitioner scope of clinical care for LTC residents in accordance with the College of Nurses (CNO) Practice Standard for Nurse Practitioners and all other applicable legislation, including assessment, ordering and interpreting diagnostics, prescribing pharmacological agents and therapeutic measures, and performing controlled acts
  • Collaboration with hospital and LTC teams to assist in identifying potential barriers and supporting with discharge strategies to reduce avoidable ED visits, decrease LOS, and minimize readmissions
  • Capacity building for LTC to develop and enhance competency for acute and chronic wounds
  • Integration of best practice guidelines and current evidence-based research into clinical practice for a select patient population
  • Evaluation of outcomes for the programs and make recommendations for change in consultation with Program Director and Manager
  • Development of linkages with external partners as appropriate to ensure continuity of care
  • Enhance communication between hospital and LTC to support the plan of care
  • In keeping with professional standards of practice, obtains and/or disseminates only that information which is relevant to meeting the client’s identified needs and with appropriate legal authorization, either in the form of signed consents or direct authorization from the client or designated substitute decision-maker.
  • Utilization of expert communication skills by being a link between the client, consultants, resources and the referral source which may be required to coordinate the provision of service to the client.
  • Participate in meetings and committees to support the planning and implementation of best practices to support needs of the residents in LTC homes
  • Participates in reflective practice as per identified college standards

Requirements:

  • Master in Nursing/Nurse Practitioner (MN-NP) or Masters of Science in Nursing with successful completion of a post Master’s Acute Care Nurse Practitioner Certificate Program from a recognized school of graduate studies required
  • Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care) required
  • 1-3 years of demonstrated experience as a Nurse Practitioner required
  • Previous experience in LTC sector preferred
  • Minimum 2-3 years direct clinical practice in complex wound management required

– Academic and clinical preparation in complex wound care management from a recognized education program an asset

  • CNA Certification in Geriatrics would be an asset
  • BLS required
  • Additional specialty education in Gerontology preferred
  • Demonstrated experience in Adult education an asset
  • Clinical specialty organizational membership: RNAO (required), CANO or ONS also preferred
  • Demonstrated strong communication and interpersonal skills, with the ability to work collaboratively with physicians, inter-professional team members, and other stakeholders
  • Possesses highly developed analytical, problem solving and critical thinking skills
  • Excellent oral, written, and presentation skills, with demonstrated competency in information and communication technology
  • Demonstrated excellent organizational and coordination skills and the ability to prioritize multiple competing workload demands
  • Demonstrates an ability to provide leadership, support and critical analysis in stressful, complex clinical and emotional situations
  • Experience in Case Management preferred.

  • Excellent attendance and a discipline free record is required (candidates are required to inform HR and the hiring manager of their attendance and discipline status)
  • Vehicle and valid Ontario driver’s license required
